‘Comedones’ is the medical term for small bumps on the skin associated with acne. They appear due to blocked pores, where the hair follicle is clogged by a combination of skin debris (keratin) and oil (sebum).

  • Blackheads are known as ‘open comedones’. They become black due to pigment at the surface of the comedo (melanin), not dirt as commonly thought
  • Whiteheads are known as ‘closed comedones’. These have no pigment at the surface and are completely blocked.

Keratin is an insoluble protein found in the skin. Combined with sebum it can form a hard plug which becomes a spot. As the keratin/sebum plug develops and grows, it may rupture the comedo wall. This is where a spot can develop into a more significant papule, pustule, nodule or cyst; becoming quite inflamed and tender.

Comedo Extraction

As comedones are the source of acne problems, some dermatologists suggest comedo extraction – which is the physical removal of comedones.

This may be done in a number of ways:

  • Use of a special comedone extractor tool
  • Steam extraction
  • Electrosurgery

The principle is that removal of the comedones should reduce inflammation and help the skin to heal.

However, it is very important that comedo extraction is done properly using sterile equipment with an experienced practitioner. This is more than an advanced version of squeezing spots! Beware as incorrect comedo extraction could even make the problem worse, potentially leading to greater inflammation and even scarring.

In addition, there is a high risk of comedones returning within a couple of months, which means that comedo extraction needs to be repeated regularly.
