Why do I have facial skin pigmentation?

Facial skin pigmentation is a very common concern as we enter autumn. It is a condition which causes the skin to appear a different colour to normal. It is characterised by patches of skin that are darker than the surrounding skin, which is lighter. The appearance of brown patches is also known as hyper-pigmentation.

Pigmentation is caused by increased levels of melanin in the skin. Melanin is the pigment that produces colour and when levels of melanin are increased, the skin appears darker in tone.

There are a number of factors that can cause melanin levels to increase:

  • Sun exposure
    This is the most common and is the prime reason why many patients look for pigmentation treatments during the autumn period. After a summer of sun exposure, pigmentation is especially noticeable as tans start to fade. The healthy glow disappears and the colour looks duller with a patchy appearance
  • Injury or inflammation
    These can cause pigmentation problems at any time of year, but can be exacerbated by the sun
  • Hormonal changes
    Surges in female hormones typically occur around pregnancy, although not exclusively so. There are also hormone changes during the perimenopause and menopause; as well as through use of the contraceptive pill. These hormonal changes can cause the skin to be more susceptible to pigmentation problems.
    Melasma or ‘pregnancy mask’ appears on the forehead, cheeks and/or upper lip.
  • Birthmarks and freckles
    These are genetic pigmentation issues, but they can be exacerbated by exposure to the sun
  • Skin type
    Certain skin types are more prone to pigmentation problems. Those with olive to darker skin (Fitzpatrick types IV-VI) are prone to developing pigmentation patches. Those with light skin (Fitzpatrick types I-II) tend to be more troubled by freckles or sun spots.

1/ Skincare products

Firstly, whatever professional treatments are provided, the latest cosmeceutical formulations are also to be considered. Skincare products with active ingredients can make an enormous difference with regular use at home. These are not ‘over the counter’ products but prescription-level preparations which have intensive benefits. For example, ingredients such as antioxidants or liquorice extract have been shown to produce a lightening effect. A short course of carefully chosen skincare could make a surprising difference.

Don’t forget to wear high factor SPF to protect your skin from the sun.

2/ Treatments for Mild Sun Damage

For skin showing symptoms such as mild pigmentation, sunspots and broken capillaries:

  • IPL treatment to eliminate little thread veins, reduce pigmentation and boost renewal
  • Mesotherapy to stimulate the skin
  • Microdermabrasion or chemical peel to exfoliate outer layers while stimulating renewal of sun damaged skin.

3/ Treatments for Medium Sun Damage

For skin with deeper pigmentation, post-inflammatory pigmentation or melasma:

4/ Treatments for Severe Sun Damage

For skin with extensive sun damage pigmentation or severe melasma which is resistant to other pigment treatments:

  • Dermamelan treatment –  specifically developed to treat hyperpigmentation, this is the ultimate intense programme. It includes a special mask applied in clinic, which contains depigmentation agents, accompanied by at home treatments before the mask is repeated.

Protection against pigmentation

If you are having treatment for pigmentation and/or sun damage, the skin will be extra vulnerable and sensitive, so it’s vital to protect it from the sun during treatment. You’ll also need to continue it afterwards to avoid the pigmentation returning.

The single most important thing you can do to avoid pigmentation problems is to protect the skin from the sun – before, during and after any treatments!

  • Wear a sunscreen with UVA and UVB protection every day, not just in summer.
  • Avoid the sun at its peak times
  • Use clothing/hat/sunglasses for further protection
  • Be especially vigilant when your skin is susceptible to pigmentation, e.g. during pregnancy
